My Vaccine Pass is an official record of your COVID-19 vaccination status for use in Aotearoa New Zealand.

Requesting a My Vaccine Pass over the phone

If you have a query about My Vaccine Pass, you can call 0800 222 478 between 8am and 8pm, 7 days a week. If your request is not urgent, please try calling after 6pm or in the weekend.

If you have not received your pass by Friday 3 December, you will still be able to access basic needs services.

If you require a pass urgently, visit a pharmacy that is offering COVID-19 vaccinations. They can help you print your My Vaccine Pass.

How to get My Vaccine Pass

There are 3 ways to get your My Vaccine Pass — online through My Covid Record, over the phone, and in person at a pharmacy.

The quickest way is through the website My Covid Record.

Find your NHI number

A National Health Index (NHI) number is a unique number that is given to each person using health and disability support services in New Zealand.

You can find your NHI number on:

  • a prescription or prescription receipt, a prescription medicine label
  • a hospital letter
  • an x-ray or test result
  • your patient portal (ManageMyHealth).

About My Vaccine Pass

My Vaccine Pass is an official record of your COVID-19 vaccination status for use within Aotearoa New Zealand. It will allow you to access certain events and venues operating under the COVID-19 Protection Framework (traffic light system).

Who can get a My Vaccine Pass

You can request a My Vaccine Pass if you are aged 12 or over and:

  • have had 2 doses of the Pfizer or AstraZeneca COVID-19 vaccine in New Zealand, or
  • been vaccinated overseas with an approved vaccine, or
  • have a medical exemption.

Your My Vaccine Pass will include your name, date of birth and a QR code. You can save it on a digital device, like your phone, or print a physical copy.

Your pass will have an expiry date 6 months after the date of issue.

Who does not need a My Vaccine Pass

People under the age of 12 years and 3 months are not required to show proof of vaccination.

Medical exemptions

If you are unable to get the COVID-19 vaccine for medical reasons, there is an exemption process to get your My Vaccine Pass.

You will need to talk to your doctor and they will apply on your behalf.

When you might need it

From Friday 3 December you may need to use My Vaccine Pass to enter events and venues as required by the traffic light system.

You may need to show it in a range of public settings. These could include:

  • events
  • hospitality
  • close-proximity businesses, such as hairdressers and gyms
  • sport
  • faith-based gatherings.

Businesses may scan the QR code on your My Vaccine Pass with the NZ Pass Verifier app. This app will confirm your vaccination status for the business. You may also be asked to show ID. It will not store your data or show any information other than your name and date of birth.

Businesses cannot accept a purple vaccination card or vaccination confirmation letter as proof of your vaccination status.

When you will not need it

You will not need to show proof of vaccination to access basic needs services like:

  • supermarkets
  • pharmacies
  • health and disability services
  • petrol stations
  • public transport — for example, buses, trains and commuter ferries
  • schools and early learning service providers
  • housing and housing support services.

If you are travelling overseas

My Vaccine Pass cannot be used for overseas travel. If you are going to another country, you will need to request an International Travel Vaccination Certificate.

NZ Pass Verifier app

The NZ Pass Verifier is a free app that businesses and organisations can use to help stop the spread of COVID-19 and keep their community safe.

At certain traffic light settings, businesses and organisations are encouraged to use the app to check customers' My Vaccine Pass.

Requirements will depend on the sector and the choices the business or organisation has made about how they will operate under the traffic light system.

Guidance about requirements for different sectors will be published on before 3 December, when New Zealand moves to the traffic light system.

If you were vaccinated in another country

If were vaccinated overseas, you must have received one of the following 8 accepted COVID-19 vaccines to get a My Vaccine Pass:

  • Pfizer/BioNTech 
  • Janssen (Johnson and Johnson) 
  • AstraZeneca (Oxford) 
  • AstraZeneca/Covishield (Serum Institute of India)  
  • Moderna 
  • Sinopharm  
  • Sinovac (CoronaVac) 
  • Covaxin (Bharat Biotech).

Your vaccination record must be added to New Zealand's COVID Immunisation Register (CIR).

You can request your My Vaccine Pass once you have received acknowledgement your record has been added.

Vaccination cards

You may receive a purple vaccination card at your first appointment. This is not official proof of your vaccination — it is to remind you when your second appointment is.
